Form SS-4: Application for Employer Identification Number

All businesses must have an employer identification number (EIN). The EIN will be used for filing taxes and other business purposes. All businesses will need to apply for this form. This form will need to be sent to the Internal Revenue Service.

The form must be completed by each employee of your business, for your records, to help you determine their federal personal withholding allowances. These forms should be kept on file for all employees.

This form is used to assign a Georgia income tax withholding identification number and/or a Georgia sales tax identification number. Upon completion mail to:

Form DOL-1A: Georgia Department of Labor Employer Status Report

This form will provide information to the Georgia Department of Labor about business activity and employment of workers. If you have not yet received a Georgia DOL unemployment tax account number, one will be assigned to you upon completion of this form. When completed mail this form to:
